...or copy it to your own workstation as file "mysubmit.htm",
and edit the value="" twice in the following lines:

| <INPUT TYPE="TEXT" NAME="address" SIZE="40" VALUE="" ><br>

You should also add a line in the <head> ... </head> section:

<base href="http://www.research.att.com/~njas/sequences">

Then you can use your own personalized file:///mysubmit.html
(e.g. as bookmark in your browser) for submissions.  If it's
in your /tmp directory use file:///tmp/mysubmit.html (etc.)
