When I taught arithmetic, the textbook I used said that "and" was only used
to separate the integer and fractional parts of numbers, e.g., 1.75 is "one
*and* three quarters."  It was never used with integers; 1001 is "one
thousand one," not "one thousand and one."  I don't remember any specific
guidance given on commas.
